Welcome to hospitalinspections.org, a website run by the Association of Health Care Journalists (AHCJ) that aims to make federal hospital inspection reports easier to access, search and analyze. This site includes details about deficiencies cited during complaint inspections at acute-care, critical access or psychiatric hospitals throughout the United States since Jan. 1, 2011. It does not include results of routine inspections or those of long-term care hospitals.

Keep in mind that this dataset does not include hospitals' responses to deficiencies, called Plans of Correction. In a Plan of Correction, a hospital may describe corrective actions, add missing information, or point out factual errors on the part of the survey team. You can request a Plan of Correction from a hospital, a state agency, or the U.S. Centers for Medicare and Medicaid Services (CMS).

This effort follows years of advocacy by AHCJ to encourage federal officials to publish this information electronically. Funding for this project is provided by AHCJ through membership dues.

Some records may be incomplete, and journalists should check with CMS before using the reports to draw hospital-to-hospital or state-to-state comparisons.

Due to variations in the way individual surveyors code reports, there is some possibility of error in these records. We recommend that questions about apparent inconsistencies be directed to the health care facilities.

Clicking on a state on the map will retrieve a list of all hospitals with their violations grouped together; choosing a state from the drop down menu will list all inspection reports separately, so a hospital may appear more than once.
